Patient Privacy

1What is your privacy statement?

Your medical record is a confidential document.

It is the policy of this Practice to maintain security of personal health information at all times, and to ensure that this information is only available to authorised staff members.

2What are Patient Health Records?

Patient health records at the practice are a combination of RACGP computerised records using Medical Director software for prescribing, letter writing, medication lists, vaccinations and reminders.

Pathology and radiology results are downloaded.

3Patient rights and feedback

All the Doctors and staff at Can-Brae Clinic are committed to providing you with a high standard of patient care. However, we acknowledge that our patients have certain rights and we aim to support these rights as much as we can. If you feel we have not met your expectations, please contact our Practice Manager by phone on 03 5332 1501.

If you feel your matter has not been resolved, you can contact the Health Ombudsman on 133 646.

Appointment

1How can I book an appointment?

Contact us today!  When booking, please indicate whether you will need a longer appointment, e.g. multiple problems, pre-employment medical, insurance or Centrelink forms etc. You may be asked by your doctor to make a further appointment if your condition is complex. Separate appointments are required for all family members needing a consultation.

2Missed Appointment

Carn-Brae Clinic requests patients to cancel or rebook their appointments if they are unable to attend for any reason with no less than a 4-hour notice. Cancelled appointments with less than 4 hours’ notice may attract an $80 fee.

If you fail to cancel or attend your appointment you will be charged with a $80 non-attendance fee.

24 hours’ notice must be given for all allied health appointments or you may attract an $80 Cancellation fee or an $80 non-attendance fee.

3What are your appointments timeslots?

Session Times:
8.30am – 6:30pm

Standard appointments are of 10 to 15 minutes duration.

4Our Fees
Carn-Brae Clinic operates as a mixed billing clinic. Our doctors provide the most up-to-date information and treatment, full payment is required on the day (please see the list below).
 
Bulk Billing is available please speak to your doctor if you have any financial difficulties, however, billing is up to the doctor’s discretion. All children under 16 years old, patients 65 years old and over, pensioners and DVA Card holders will be bulk billed.
 
It is our aim to keep providing our community with exceptional care and ensure that we continue to operate and provide the best service.

Communication

1What about telephone access to doctors?

Your doctor cannot give you their full attention if they are interrupted by phone calls. All medical matters must be dealt with by a consultation rather than by phone. If however, you are asked by your doctor to ring, our receptionists can either put you through to the doctor, or take down your details and the doctor will call at their earliest convenience.

2What about emergencies?

Phone immediately and explain the situation to the receptionist who will either put your call straight through to the doctor, arrange for the patient to be seen as soon as possible, or arrange other emergency care such as ambulance, hospital assessment or admission.

3Recall and reminders

Our Practice is committed to preventative care. Your Doctor will seek your permission to be included on our Reminder System. We may issue you with a Reminder Notice from time to time, offering you preventative health services appropriate to your care.

If you do not wish to be part of this system, please advise reception or your doctor. In the case of abnormal test & imaging results, this Practice has a system of patient recall, whereby you will be contacted by telephone or post.

4Email

Carn-Brae Clinic does not communicate with patients by email. We do not accept emails containing patient’s personal information, any emails received that contain a patient’s personal information will be immediately deleted.
